Application Instructions

Step #1: Submit this application. Please make sure it is filled out completely, including the Release and Risk Agreement and the Parent’s Consent and Release. Note: Items marked with an asterisk (*) are required.

Step #2: After submitting the application, a Confirmation page will appear along with a link to a payment page. We accept Visa, Mastercard, AMX, and Discover cards. A 50% deposit is expected for the session you select.

Special Needs::	Please tell us of any physical, mental or emotional problems we should take into consideration in our instruction of this student.* *Failure to inform us may result in non-refundable termination of camp fees. It will help if we know ahead of time of a child’s particular needs so that their camp experience is both a fulfilling and memorable one.
Knollwood Farm LTD Release and Risk Agreement It is mutually agreed that Knollwood Farm LTD assumes all risk to the horses from time used by the undersigned customer and the customer assumes all risk of injury to himself or herself and releases Knollwood Farm LTD from all liability thereafter. Notice: A person who is engaged for compensation in the rental of equines or equine equipment or tack or in the instruction of a person in the riding or driving of an equine or in being a passenger upon an equine is not liable for the injury or death of a person involved in equine activities resulting from the inherent risks of equine activities, as defined in section 895.481(1)(e of the Wisconsin statutes.

Parent’s Consent and Release The undersigned parent(s) of the above-named customer, a minor, hereby consents that said minor may participate in the teaching and training program of Knollwood Farm LTD and releases Knollwood Farm LTD from all liability for any injury that might occur to said minor during the course thereof and we do agree to indemnify and hold harmless Knollwood Farm LTD for any and all claims and demands that might or could be made by, for, or on behalf of said minor mident to any injury occasioned by him or her in the course of such teaching and training.
